Income and Expense Form

Every court in Illinois requires parties to a divorce to share certain information with the other side.  Before any “discovery” efforts are initiated, the parties are expected to complete an affidavit in which they provide information about their income and expenses. The attached form is used in many counties in Illinois and can be easily adopted to the requirements of any Illinois court. It will provide your lawyer with critical financial information so your case can move forward quickly and smoothly. More information may be required later.
